To reserve a space at CHC for your event, please complete and submit the following form.

(Form submission does not guarantee approval.)

 

The event request form below is to be submitted when church space and/or tech resources are required.

if there are graphics or communication needs as well, please submit a communication request form.

(Only CHC approved videographers/technicians will be used. Additional charge may apply. Please have person with video and/or presentation arrive 30 minutes prior to event to meet with technical crew.)
TV/DVD *
(If 24+ people will attend, large screen projector will be needed.)
Staging *
(Must submit a room diagram for approval/setup.)
Lighting *
Anything other than standard ambient lighting. (Only CHC approved lighting technicians will be used. Additional charge may apply.)
Security *
